If you were seen on or after July 1, 2026, your medical records may currently be maintained by two different entities due to the recent merger. We are actively working to move and consolidate records into the new Epic system and expect the records transition to be fully completed by September 2026.
During this transition period, records may be provided by more than one source. Once a request is received, our staff will review it and direct it to the appropriate records source. Processing times may vary while records are being moved and merged.
Instructions for Requesting Medical Records
To help us protect patient privacy and ensure records are released appropriately, PENTA patients requesting medical records should use one of the options below.
- Request records in person. Patients may come to the office to sign a medical records release form in person.
- Call to begin the request process. Patients may call the main line at (336) 768-3361 to ask questions or begin the medical records request process.
- Fax a written request. Patients or requesting offices may fax a written request to (336) 768-4131. Faxed requests should include the patient’s name, date of birth, contact information, the records being requested, where the records should be sent, and any required authorization or release form.
